Here's what you're being asked to join.

What you'll see: Appointments, bills, to-dos and notes for the person you'll be helping, all in one place. No sign-up. He or she doesn't need anything.
What you won't see: Medications and health details. Those stay on the phone they were typed into.
What it costs you: Nothing. Whoever invited you pays for the family plan.
